Who qualifies, and what will it cost?

Eligibility is simpler than it sounds

A patient may qualify when a doctor certifies that life expectancy could be six months or less if the illness runs its normal course, and the patient chooses comfort-focused care instead of treatment aimed at a cure.

The diagnosis does not have to be cancer. Sovereign serves people with any qualifying terminal illness, including heart disease, lung disease, kidney disease, stroke, and dementia.

For most Medicare families, the care costs $0

You pay nothing for Medicare-covered hospice services from a Medicare-approved provider. The care team, equipment, supplies, and medications related to comfort and symptom management are included.

A copay of up to $5 can apply to certain comfort medications. Inpatient respite care may carry 5% coinsurance. Sovereign Hospice has been Medicare-certified since 2019 and accepts Medicaid.

Does Medicare cover hospice care?

Yes. Hospice care is 100% Covered by Medicare. You pay nothing for Medicare-covered hospice services from a Medicare-approved provider. A copay of up to $5 can apply to certain comfort medications, and you may pay 5% of the Medicare-approved amount for inpatient respite care. Sovereign Hospice is Medicare-certified and accepts Medicaid as well.

Is choosing hospice giving up?

No. Hospice means choosing more support, not less: comfort, dignity, and time together, with a full care team behind you. The focus shifts from treatment aimed at a cure to managing symptoms and making each day as comfortable as possible.

What if we change our minds?

You can stop hospice at any time, for any reason, including to return to treatment aimed at a cure. Choosing hospice is not a one-way door. Your family stays in control of the decision.

How quickly can care begin?

Our phones are answered 24 hours a day, every day. In urgent situations, care can often begin within 24 hours of your first call.

Does 24-hour home hospice mean a nurse stays in the home around the clock?

Not usually. Routine hospice care includes scheduled visits based on the patient’s care plan, with phone support available day and night. Medicare also provides for continuous home care during brief periods of crisis when it is needed to keep symptoms under control at home.

Is hospice only for a person’s final days?

No. Medicare hospice eligibility is based on a doctor’s certification that life expectancy may be six months or less if the illness runs its normal course. Care can continue beyond six months when the patient remains eligible and is recertified.

What does hospice actually provide?

Medicare-covered hospice includes an interdisciplinary care team, nursing care, hospice aide services, medications for pain and symptom management, medical equipment and supplies, counseling, and other services related to the terminal illness and care plan.

Can hospice care be provided in a nursing home or assisted living community?

Yes. Sovereign Hospice provides care in private homes, nursing homes, assisted living, memory care, senior living, independent living, retirement communities, and residential care facilities.

What is the difference between hospice and palliative care?

Hospice is comfort-focused care for eligible patients who are no longer pursuing treatment aimed at a cure.
